I remember defintiely two times in shotgun. Might have been more but not many if any and they were very late in the game when it was pretty much over with anyway. Sort of Henning tossing them in so he could say we did try it and still lost.

 

looking at the PBP, NFL.com suggests we ran it exactly 5 times, all after Kasay's score, which was the game ending drive.
